Providing support

Returning to everyday life after inpatient discharge can be difficult. Locating providers, engaging resources, and staying on top of care can be overwhelming for clients, further perpetuating a cycle of crisis. LEAN’s goal is to break this cycle by creating a bridge between inpatient care and outpatient care.
LEAN builds this bridge by supporting the individual and their family during and after discharge through a partnership with the referring agency that prepares the participant for return to everyday life.

Through this partnership, we work with the individual and an extensive statewide network (professional and non-professional services) to assist in monitoring individual needs, identifying supports, and connecting them with resources in their area.

Supporting Youth Through Follow-Up Care

LEAN provides support to participants with a range of diagnoses and comorbidities. These diagnoses may include:

  • Major depressive disorder
  • Generalized anxiety disorder
  • Attention-deficit/hyperactivity disorder (ADHD)
  • Other diagnoses

While these disorders are common amongst our treatment populations, we also treat participants with other inappropriate behaviors and comorbid mental health diagnoses, such as:

  • Alcohol-use disorder
  • Cannabis-use disorder
  • Opioid-use disorder
  • Tobacco-use disorder
  • Other substance-related disorders

Comprehensive Care

LEAN is not just another case management service – we provide comprehensive follow-up care throughout Louisiana regardless of the individual’s location and intensity of symptoms.

We are there for the client during and after the discharge process to ensure that they are informed and supported every step of the way. And in the event that the client experiences a future crisis, our partnership with the referring agency allows us to quickly re-connect them with the help that they need.

By engaging available supports at every level, participants have the resiliency to address triggers head on, cope with stressors, and prepare for crises in the future. The result is a simpler, safer transition back home.
